Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.

This information can be obtained under a FOI request in writing to the Practice Manager.  The Practice Manager can be contacted at Practice.manager.w93019@wales.nhs.uk

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

We do not publish our annual salaries; in general we follow NHS Agenda for Change.

Class 3 – What our priorities are and how we are doing

  • Working closely with our Monmouthshire North Cluster colleagues to develop services for patients across North Monmouthshire. You can find out more here: Monmouthshire North Cluster
  • Working towards achieving required standards within the Welsh Government QAIF (Quality and Information Framework), Access Standards, Medicines Management, Patient Safety, CGPSAT, IG toolkit and Healthier Wales
  • The practice has not had an inspection for a while but is probably due to be inspected by HIW (Health Inspectorate Wales) in the near future. This report will then be published on this site
  • Both of our buildings have recently been updated to meet pandemic guidelines, including new flooring which was a high priority.  We continue to seek funding and approval for plans to make improvements to our St Briavels surgery site. 
  • We hope to renew our PPG (Patient Participation Group) when possible and will publish minutes of future meetings on this site also. 

Class 4 – How we make decisions

The Practice has a number of teams that are made up of all GP partners, salaried GPs, Practice Manager and Nursing team. 

The management and clinical teams meet on a regular basis.  These meetings cover a wide variety of topics including confidential patient information which means we are not able to provide copies of the minutes taken.  

Class 5 – Our policies and procedures

We have many policies and procedures that govern and guide the work of the Practice and are constantly developing and reviewing these. Information about these can be obtained via the Practice Policies page.  For more information on accessing medical records or raising a complaint, see here.

Class 6 – Lists and Registers

In order to meet national standards we hold confidential and sensitive patient lists, including disease registers and palliative care lists.  These are not publicly available as they contain confidential information.
